Quilt Raffle!

This quilt was hand made and generously donated by one of our parents, Beverly Wilson, who has over 30yrs. of quilting experience. She made this particular masterpiece by generator light in the aftermath of the tornado that touched down in Genoa, WV in March of 2012.
Chances to win are only $5 each and tickets may be purchased here at Mainstream in Lavalette, or at our 5K race at the old C-K High School in Kenova on June 8th. All proceeds will go to the Building Fund. The drawing date is TBA, the winner will be notified by phone, and we can deliver! If there are any questions, please call us at 304-522-1945.
Applying for Waiver:
The process to apply for Title XIX Waiver has changed. The application is to be completed and submitted to Charleston (the address at the bottom of the application). Within 5 days, the Charleston Office will contact the family with a list of psychologists who can complete the evaluation. A final determination will be presented to the family within 90 days. Unfortunately, one thing about the Waiver application process has not changed....the wait list! If you don't start the process you can never get your waiver slot.
You can download the application here in
MS Word or
PDF formats (click desired icon).
