A Special Message From The Executive Director:
Dear Parents, Guardians and Consumers, I hope all of you are enjoying the spring season and anticipating the fun activities it brings. I wanted to give you a little advance notice of some fun and exciting happenings.
June 15, 2012 Mainstream Academy will be celebrating its 5th anniversary. There will be an open house from 1:00p.m. - 3:00p.m. Refreshments will be provided. The Academy will be open to the public. Family and friends are encouraged to attend as we celebrate this important milestone. RSVP is not required but would be appreciated. Contact Pam Hodge at 304-522-1945.
On July 19th we will be hosting Christmas in July. It will be held at Dreamland Pool. We will provide hot dogs, chips and drinks. Ed Laymen will be providing music. We really had a great time last year. We had the pool to ourselves. I hope you will be able to attend.
In keeping with the Christmas theme, our annual Christmas Party will be held December 8, 2012. I am hoping to have it at Kellogg Elementary again but that is not confirmed.
To help us help others, we've created a flyer to be distributed where allowed, at places of business and locations visible to potential members and their families. Simply download the PDF here, and print as needed.
If you have any questions about these gatherings or an idea for something else please let me know.

Applying for Waiver:
The process to apply for Title XIX Waiver has changed. The application is to be completed and submitted to Charleston (the address at the bottom of the application). Within 5 days, the Charleston Office will contact the family with a list of psychologists who can complete the evaluation. A final determination will be presented to the family within 90 days. Unfortunately, one thing about the Waiver application process has not changed....the wait list! If you don't start the process you can never get your waiver slot.
You can download the application here in
MS Word or
PDF formats (click desired icon).
