Down Syndrome Network of West Virginia - Buddy Walk

From The Mighty Quinn's Crew:

"We are walking to show our support for our child and more than 400,000 other individuals with Down syndrome in the U.S. Every dollar we raise brings us one step closer to ensuring that our child and every individual will have the opportunity to reach their potential.

Our fundraising efforts will support local programs and services, as well as National Down Syndrome Society advocacy and public awareness initiatives that benefit all individuals with Down syndrome."

Applying for Waiver:

The process to apply for Title XIX Waiver has changed. The application is to be completed and submitted to Charleston (the address at the bottom of the application). Within 5 days, the Charleston Office will contact the family with a list of psychologists who can complete the evaluation. A final determination will be presented to the family within 90 days. Unfortunately, one thing about the Waiver application process has not changed....the wait list! If you don't start the process you can never get your waiver slot.

You can download the application here in

MS Word or PDF formats (click desired icon).
